Forms are available in the link below if you wish to begin filling out the form ahead of time. The museum will also have printed forms available at the event. Representatives from the MSU Quilt Index will be at the museum to assist in completing the forms.
The event is free and will be held in the museum auditorium. If you wish to go into the galleries, please purchase an admission ticket in the gift store.
